Full Job Description Qualifications:

Must be licensed by the Physical Therapy Board of California. Must possess current CPR and First Aid certification (or within 6 months of hire) in addition to a TB clearance and health examination from a physician (at time of hire or within 7 days of hire). Must be a team player to ensure close, clear and timely communication with a multidisciplinary team. Must possess intermediate computer skills and be comfortable with electronic records keeping and updating.

Specific Duties:

Providing treatment according to care plan. Treatment of participants to achieve and maintain maximum performance using physical means such as exercise, massage, heat, sound, water, light or electricity. Recording and signing treatments each day, noting method of treatment and reaction of participant and narrative notes (as needed). Also responsible for signed and dated quarterly progress notes. Instruction of other health team personnel and family members in methods of maintained and improving functioning. Supervision of maintenance program designed by an occupational and/or physical Therapist.
