Job Description

South Pacific Rehab Services is seeking a compassionate Physical Therapist Assistant to provide high-quality, resident-centered rehabilitation in skilled nursing and post-acute settings. Working under the direction of a Physical Therapist, you will implement individualized treatment plans, assist with gait and balance training, therapeutic exercise, and functional mobility, and document progress in accordance with regulatory standards. Responsibilities Implement physical therapy treatment plans under direction of supervising PTProvide therapeutic exercises, gait, balance, and functional mobility training Assist with transfer training, wheelchair mobility, and positioning programs Monitor, document, and report patient progress and response to treatment Ensure accurate, timely, and compliant clinical documentation Collaborate with PTs, nursing, and interdisciplinary team on care plans Educate patients, families, and caregivers on exercises and safety Adhere to ethical standards, company policies, and regulatory requirements Maintain safe, clean treatment areas and proper use of rehab equipment Participate in in-services, continuing education, and quality improvement Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility training Transfer and wheelchair training Rehab documentation (EMR) Knowledge of SNF/post-acute regulations Patient and family education Safety and fall-prevention techniques Vital signs monitoring during therapy Interdisciplinary team collaboration
