Job Summary We are currently seeking a compassionate and dedicated Physical Therapist Assistant (PTA) to join our team at a well-established Skilled Nursing Facility in Healdsburg, CA . In this role, you will work under the supervision of a licensed Physical Therapist to provide high-quality rehabilitative care, helping patients improve mobility, restore function, and achieve their highest level of independence. Key Responsibilities Provide physical therapy treatments as directed by the supervising Physical Therapist. Assist patients with therapeutic exercises, gait training, balance activities, and mobility programs. Monitor, document, and report patient progress and responses to treatment. Educate patients and caregivers on home exercise programs and safe mobility techniques. Support the implementation of individualized treatment plans. Maintain accurate and timely clinical documentation. Ensure compliance with federal, state, and facility policies and regulations. Collaborate with the interdisciplinary healthcare team to deliver exceptional patient care. Qualifications Graduate of an accredited Physical Therapist Assistant (PTA) program. Active California PTA License (or eligible). Current CPR/BLS certification (preferred). Previous experience in a Skilled Nursing Facility or long-term care setting is preferred.
