Job Description

South Pacific Rehab Services is seeking a dedicated Physical Therapist Assistant to join our Healthcare & Medical Services team. In this role, you will provide direct patient care under the supervision of a Physical Therapist in skilled nursing and post-acute settings. Responsibilities include implementing treatment plans, documenting progress, and collaborating with an interdisciplinary team to promote functional recovery and quality of life. Responsibilities Deliver individualized PT interventions under PT supervision in SNF and post-acute settings Assist with evaluations by performing objective measures and reporting patient status to the PTImplement treatment plans focused on mobility, strength, balance, and functional independence Document treatments, progress notes, and patient responses accurately in the EMREducate patients and families on exercises, safety, and functional strategies for carryover Collaborate with nursing and interdisciplinary team to support resident-centered goals and discharge plans Maintain compliance with state, federal, and facility regulations and ethical standards Participate in team meetings, in-services, and ongoing clinical education Contribute to a clinician-centered, collaborative culture that prioritizes quality outcomes Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility training Modalities application (e-stim, ultrasound, heat/cold) Patient transfer and safety techniques Documentation in EMRCare plan implementation Therapeutic activity training (ADLs) Range of motion techniques Interdisciplinary team collaboration
