Job Description
South Pacific Rehab Services is seeking a dedicated Physical Therapist Assistant to join our Healthcare & Medical Services team. In this role, you will provide direct patient care under the supervision of a Physical Therapist in skilled nursing and post-acute settings. Responsibilities include implementing treatment plans, documenting progress, and collaborating with an interdisciplinary team to promote functional recovery and quality of life. Responsibilities Deliver one-on-one physical therapy treatments under the direction of a Physical Therapist Implement individualized care plans to improve mobility, strength, and function Document treatment sessions, patient progress, and outcomes in the EMR system Educate patients and families on exercises, safety, and functional strategies Assist with discharge planning and goal setting within an interdisciplinary team Communicate changes in patient status promptly to supervising PT and nursing staff Ensure safe use of equipment and adherence to infection control and facility policies Contribute to a clinician-centered, ethical, and patient-first culture Required Skills Therapeutic exercise implementation Gait training and balance interventions Functional mobility training Use of rehab equipment and modalities Documentation in EMR systems Patient and family education Care plan implementation under PT direction Vital signs monitoring and basic patient assessment Safe transfer and body mechanics techniques Interdisciplinary team collaboration
