North Sacramento As a Home Health PTA, you will: Provide skilled physical therapy interventions as ordered by the physician and under the direction and supervision of a licensed Physical Therapist. Support patients in improving mobility, balance, gait, strength, and functional independence while helping minimize physical limitations. Educate and instruct patients and families/caregivers on home exercise programs, balance and gait training, and strategies to support continued progress. Monitor patient condition and progress, communicating changes and concerns with the supervising Physical Therapist, physician, case manager, and care team. Assess the home environment and recommend appropriate modifications or strategies to promote patient safety and independence. Maintain accurate, complete, and timely clinical documentation in accordance with Lorian policies, procedures, and regulatory requirements. Collaborate with the interdisciplinary care team, participate in education and in-service programs, and provide guidance to other healthcare personnel when appropriate. Required Qualifications Graduate of a physical therapy assistant school approved by the Council of Medical Education of the AMA in Association or its equivalent. Licensed/registered to practice as a PTA in the state of practice. Valid driver's license and insured personal vehicle (per Lorian requirements). Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting. Possesses and maintains current CPR certification. This position is based on a pay per visit compensation model. Pay per visit ranges from $45 to $60. Visit commensurate with experience and depending on type of visit.
