QUALIFICATIONS
Current license to practice as a Physical Therapy Assistant in the state(s) of employment. Graduate of an accredited physical therapy assistant program approved by the American Physical Therapy Association. Minimum of six (6) months of experience as a Physical Therapy Assistant. Demonstrated written, verbal communication skills, and interpersonal skills. Licensed driver with an automobile insured in accordance with state/Agency requirements. Reliable transportation with a good driving record.
ESSENTIAL FUNCTION/AREAS OF ACCOUNTABILITY 1.
Provides physical therapy services and education/training to clients in their homes following the physician's Plan of Care and the specific functions identified in the therapy care plan. a. Completes visits in a timely manner and in accordance with Agency policy. b. Documents all services provided, observations, client progress, and recommendations on designated forms and submits to the agency within seven (7) days of providing service or per Agency policy. c. Reports changes/concerns or schedule changes to the therapist and/or the nursing supervisor on the day of the observed change. 2. Participates in interdisciplinary activities per Agency guidelines. a. Attends client care conferences as needed to provide information to other members of the team. b. Promotes referrals to other disciplines as appropriate. c. Attends staff meetings and continuing education programs. d. Participates in the Agency quality improvement activities pertinent to role of Physical Therapy Assistant. 3. Promotes personal safety and a safe environment for clients and coworkers. a. Demonstrates knowledge of safety and infection control practices by compliance with Agency policies and procedures. b. Recognizes and reports potentially unsafe situations. c. Demonstrates safe practice in the use of equipment. d. Assesses environment and initiates actions to prevent accidents and promote safety. 4. Maintains confidentiality in all aspects of the job. a. Does not discuss client or staff identities, conditions, or circumstances in public or with others who are not professionally involved with the situation/person. b. Protects written confidential documents in a manner that prevents unauthorized access. c. Adheres to all Agency policies about client privacy and security of information. 5. Performs job in compliance with Agency policies and procedures, Medicare, CHAP, and/or Joint Commission standards and professional and community standards. a. Accepts responsibility for personal and professional development. Identifies learning/developmental needs. 6. Performs additional duties and responsibilities as deemed necessary.
