Skilled Nursing Facility Responsibilities Deliver PTA services to residents under licensed PT supervision in a skilled nursing facility setting Implement and monitor approved treatment plans; document progress and adjust as directed with the supervising PT Lead therapeutic exercises, gait training, balance activities, functional mobility, transfers, and repetition-based programs Apply modalities and manual techniques within scope as permitted; participate in functional capacity evaluations as assigned Educate residents and families on home exercise programs, safety, fall prevention, and activity modification Maintain timely documentation of patient progress in Rehab Optima (NetHealth) as applicable; communicate clearly with the PT and rehab team Collaborate with nursing and therapy colleagues to coordinate care and ensure patient safety and quality outcomes Requirements Active California Physical Therapy Assistant license
Education:
