South Pacific Rehab Services is seeking a compassionate Physical Therapist Assistant to join our Healthcare & Medical Services team in a skilled nursing and post-acute setting. The PTA will deliver one-on-one, resident-centered care, implementing treatment plans under the direction of a Physical Therapist to improve mobility, function, and quality of life. Responsibilities include therapeutic exercises, gait training, documentation, and collaboration with an interdisciplinary team. Responsibilities Provide direct physical therapy interventions under supervision of a licensed Physical Therapist Implement treatment plans focused on mobility, strength, balance, and functional independence Assist with gait training, transfer training, and therapeutic exercises Monitor, document, and report patient progress in accordance with regulatory and facility standards Collaborate with PTs, OTs, nursing, and other interdisciplinary team members Educate residents and families on home exercise programs and safety strategies Ensure safe use and maintenance of therapy equipment Participate in care conferences and contribute to individualized care plans Adhere to ethical, regulatory, and infection control standards in a skilled nursing/post-acute setting Engage in continuing education and in-service training to maintain and enhance clinical skills Required Skills Physical therapy interventions Gait and transfer training Therapeutic exercise programming Neuromuscular re-education Functional mobility training Clinical documentation (EMR) Patient and family education Safety and fall prevention Knowledge of Medicare and SNF regulations Interdisciplinary care collaboration
