Responsibilities include:
Provide skilled PTA treatment to patients in their homes Implement the Physical Therapist's established plan of care Perform therapeutic exercises, functional mobility training, gait training, transfer training, balance activities, strengthening, and other interventions within the PTA scope of practice Educate patients, caregivers, and family members regarding prescribed exercises, mobility techniques, fall prevention, safety, and home exercise programs Monitor patient response to treatment and communicate changes in condition or progress to the supervising Physical Therapist and appropriate care team members Reinforce proper use of assistive devices and durable medical equipment as appropriate Assist patients with improving strength, mobility, balance, endurance, safety, and overall functional independence Complete accurate and thorough clinical documentation in accordance with agency, payer, and regulatory requirements Submit documentation within required timeframes Maintain communication with supervising PTs, agency staff, physicians, caregivers, and other interdisciplinary team members as appropriate Participate in care coordination and discharge planning Maintain patient confidentiality and comply with HIPAA requirements Follow all applicable federal, state, agency, and professional standards Requirements Current California Physical Therapist Assistant (PTA) license in good standing Previous Home Health experience required Graduate of an accredited Physical Therapist Assistant program Current CPR/BLS certification Valid driver's license Reliable transportation and current automobile insurance Professional liability insurance Strong clinical judgment, communication, organization, and time-management skills Ability to work independently while maintaining effective communication with the supervising Physical Therapist and interdisciplinary team Comfortable using electronic medical record (EMR) systems and completing documentation electronically Ability to meet home health documentation and visit submission requirements Ideal Candidate We're looking for a PTA who is experienced in Home Health, organized, dependable, communicative, and comfortable managing a mobile caseload.
