We are looking for a compassionate Physical Therapist Assistant to join our growing Home Health team serving Los Angeles and surrounding communities. Whether you are an experienced Home Health therapist or transitioning into Home Health for the first time, our team is here to support your success. Follow the established Plan of Care. Perform therapeutic exercise, gait, balance, transfer, and mobility training. Monitor and communicate patient progress or changes to the supervising PT. Complete accurate and timely clinical documentation. Educate patients and caregivers on safety and home exercise programs. Maintain reliable communication with patients and the clinical coordination team. Follow HIPAA and all professional/home health standards. Maintain current PTA license and required credentials. Travel to patients within the agreed service area. Qualifications Current California Physical Therapist Assistant License Current driver's license and reliable transportation Excellent communication and organizational skills Home Health experience preferred but not required New Home Health therapists are encouraged to apply •We believe therapists should be supported, not micromanaged•Our coordinators help with scheduling and communication, our leadership team is accessible when questions arise, and we work hard to create a positive environment where clinicians can focus on patient care.
