Job Description
South Pacific Rehab Services is seeking a compassionate Physical Therapist Assistant to provide hands-on therapy in skilled nursing and post-acute care settings across Hawaii. In this collaborative, patient-centered role, you will implement individualized treatment plans under the direction of a Physical Therapist, focusing on improving mobility, strength, and functional independence. You'll document progress, educate patients and families, and coordinate with an interdisciplinary team. Responsibilities Implement physical therapy treatment plans under the supervision of a Physical Therapist. Provide therapeutic exercises, gait training, and functional mobility interventions. Monitor, document, and report patient progress in accordance with regulatory and facility standards. Educate patients and families on exercises, safety, and use of assistive devices. Collaborate with an interdisciplinary team to support patient-centered goals. Ensure safe, clean, and compliant treatment environments. Assist with discharge planning and home exercise program instruction. Participate in professional development, training, and quality improvement initiatives.
