You will work with a team of experts in rehabilitation of neurological and cognitive conditions serving patients in their homes. This team consists of Physical and Occupational Therapy to provide comprehensive therapy to patients. Please note that this is outpatient therapy delivered to the patient's home in a senior living community, NOT home health. Responsibilities Implement therapeutic exercises and techniques to improve mobility and function under the supervision of a licensed physical therapist onsite. Educate patients and families on managing symptoms and enhancing quality of life. Collaborate with healthcare teams to coordinate care and discharge planning. Maintain accurate patient records and documentation in compliance with medical standards. Minimum Qualifications You are a PTA Licensed in CA 1+ years of experience in geriatrics preferred CPR and BLS certification
