Position Summary:

We are currently seeking a licensed and motivated Physical Therapist Assistant (PTA) to join our growing home health team. Under the supervision of a licensed Physical Therapist, the PTA will provide therapy services to patients in their homes to help improve mobility, strength, and independence.

Responsibilities:

Provide in-home physical therapy treatments as directed by the supervising PT Assist in the development and implementation of individualized care plans Monitor patient progress and report changes to the PT and care team Educate patients and caregivers on exercises and mobility techniques Document services provided and maintain accurate records Communicate effectively with team members, patients, and families Travel to patient homes within your assigned territory Educate patients and their families about exercises, care plans, and discharge planning to ensure continuity of care. Collaborate with healthcare professionals to coordinate patient care and optimize treatment outcomes. Maintain accurate records of patient treatments and progress using medical terminology. Support geriatric patients by adapting therapy techniques to meet their specific needs. Skills Current license as a Physical Therapist Assistant (PTA) in the state of California Minimum of 1 year of experience in a clinical or home health setting preferred (new grads welcome to apply!) CPR certification Valid driver's license and reliable transportation Strong communication and organizational skills Ability to work independently and manage time effectively

$90.00 - $110.00 per hour
