Job Description
Tender Touch Rehab Services seeks a compassionate Physical Therapist Assistant to support therapy services for seniors in skilled nursing and long term care settings. Working under the direction of a Physical Therapist, you will implement individualized treatment plans, lead therapeutic exercises, functional mobility and balance training, and document progress in accordance with regulatory and facility standards. You'll collaborate with an interdisciplinary care team, educate patients and families, and help promote safety, independence, and quality of life in a supportive, patient centered environment with opportunities for growth. Responsibilities Implement treatment plans under supervision of a licensed Physical Therapist Provide therapeutic exercise, gait, balance, and functional mobility training Assist with transfer training and use of assistive devices for seniors Observe, measure, and document patient progress and response to treatment Maintain accurate, timely, and compliant clinical documentation Educate patients, families, and caregivers on home programs and safety Collaborate with nursing, rehab, and medical staff on care plans Contribute to discharge planning and functional goal setting Ensure safe use and upkeep of therapy equipment and treatment areas Participate in staff meetings, in services, and ongoing clinical education Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility and transfer training Geriatric rehabilitation techniques Skilled nursing/long-term care rehab practices Clinical documentation (EMR) Patient and family education Assistive device fitting and training Pain management modalities as permitted by state law Interdisciplinary team collaboration
