Job Description

Physical Therapist Assistant (PTA) - Outpatient Physical Therapy in the Home Visiting Nurses of the Lower Valley is seeking a Physical Therapist Assistant to join our growing outpatient physical therapy program. This is an outpatient physical therapy position in which services are delivered directly in the patient's home. It is different from traditional Medicare home health. Patients do not need to meet homebound criteria, and treatment is provided under the outpatient therapy benefit rather than the home health benefit. The PTA will work under the supervision and direction of a licensed Physical Therapist and provide skilled treatment in accordance with the established plan of care. This position offers therapists the opportunity to provide one-on-one care in the patient's own environment while avoiding the pace and volume commonly associated with a traditional outpatient clinic setting.
Responsibilities include:
Providing skilled physical therapy treatment in patients' homes Carrying out the established PT plan of care Communicating patient progress and changes in status to the supervising PT Completing timely and accurate documentation Providing patient and caregiver education as appropriate Maintaining communication with the rehabilitation team regarding patient needs and progress
Requirements:
Current Connecticut PTA license Graduate of an accredited Physical Therapist Assistant program Valid driver's license and reliable transportation Strong communication, organizational, and documentation skills Ability to work independently in the home setting Therapists with outpatient, orthopedic, geriatric, or general rehabilitation experience are encouraged to apply.
Pay:
$35.00 - $45.00 per hour
