Physical Therapist Assistant PTA Home Health Help Patients Build Strength and Independence at Home Elara Caring is hiring a compassionate Physical Therapist Assistant to serve home health patients throughout lower Fairfield County. The territory extends from Norwalk to the New York state line , including Stamford and Greenwich . As a Home Health PTA, you will provide personalized therapy in patients' homes under the supervision of a Physical Therapist. You will help patients improve their strength, mobility, balance, safety, and confidence while building meaningful one-on-one relationships. Meaningful one-on-one patient care Work autonomy and flexible scheduling Supportive and collaborative therapy team Comprehensive onboarding and mentorship Tuition reimbursement for eligible full-time employees Continuing education opportunities at no cost Opportunities for professional growth and advancement Medical, dental, and vision insurance 401(k) with employer match Paid time off and paid holidays Family and pet bereavement Pet insurance What You'll Do Provide physical therapy treatments according to the established Plan of Care Work under the direction and supervision of a licensed Physical Therapist Help patients improve mobility, strength, balance, endurance, and independence Monitor patient progress and promptly report changes in condition to the supervising Physical Therapist Review the Plan of Care before each visit Provide treatment according to physician orders and established clinical standards Teach patients and caregivers how to safely complete home exercise programs Educate patients on the safe use of walkers, canes, wheelchairs, prosthetics, orthotics, and other assistive devices Reinforce patient goals and encourage participation in therapy Complete accurate and timely clinical documentation Collaborate with nurses, therapists, physicians, and other members of the care team What You'll Bring Graduate of an accredited Physical Therapist Assistant program Current and unrestricted Connecticut Physical Therapist Assistant license At least six months of experience as a Physical Therapist Assistant preferred Strong communication, organization, and documentation skills Ability to work independently in patients' homes Willingness to travel throughout Norwalk, Stamford, Greenwich, and the assigned coverage area Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations. Must be able and willing to travel 50% Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Make Every Step Forward Count Bring your therapy skills directly to patients throughout lower Fairfield County.
