Physical Therapist Assistant (PTA) - Per Diem Leeway | Skilled Nursing Facility Leeway is seeking a Per Diem Physical Therapist Assistant (PTA) to support our rehabilitation team in providing high-quality, resident-centered care. This position is ideal for a motivated, compassionate PTA who is committed to helping residents achieve their highest level of mobility and independence. Position Type Per Diem / As Needed Flexible scheduling Key Responsibilities Provide physical therapy treatments to residents under the direction and supervision of a licensed Physical Therapist Assist with implementing individualized plans of care focused on improving strength, mobility, balance, and functional independence Monitor resident response to treatment and report progress or concerns to the supervising Physical Therapist Document daily treatment notes and participation in accordance with facility policies and regulatory requirements Collaborate with nursing, therapy, and interdisciplinary team members Educate residents and caregivers on safe mobility techniques and therapeutic exercises Adhere to all safety, infection control, and compliance standards Qualifications Graduate of an accredited Physical Therapist Assistant program Current Connecticut PTA license (required) Skilled nursing or long-term care experience preferred, but not required Strong communication, documentation, and teamwork skills Ability to follow established plans of care and provide compassionate resident support What We Offer Competitive per diem compensation Supportive and collaborative work environment Flexible scheduling Opportunity to make a meaningful impact in resident rehabilitation and quality of life
