Job Description

Tender Touch Rehab Services is seeking a compassionate Physical Therapist Assistant to join our Healthcare & Medical Services team serving seniors in skilled nursing and long-term care settings. In this patient-centered role, you'll deliver individualized treatment plans under the direction of a Physical Therapist, focusing on mobility, strength, balance, pain management, and functional independence. Collaborate closely with an interdisciplinary team, document progress, educate patients and families, and contribute to clinical excellence in a supportive culture that values mentorship, ongoing education, and professional growth in post-acute care. Responsibilities Provide physical therapy interventions under supervision of a licensed Physical Therapist Implement and progress individualized treatment plans for senior patients Assist with gait training, balance, strength, and functional mobility activities Monitor and document patient progress, outcomes, and tolerance to treatment Educate patients, families, and caregivers on exercises and safety strategies Collaborate with an interdisciplinary team to support care plans and discharge goals Maintain accurate, timely, and compliant clinical documentation Ensure safe use of equipment and adherence to infection control and facility policies Participate in care conferences and communication with nursing and medical staff Support quality improvement, evidence-based practice, and professional development initiatives Required Skills Therapeutic exercise implementation Gait training and balance therapy Functional mobility and transfer training Use of assistive devices and durable medical equipment Therapeutic modalities (e.g., heat, cold, electrical stimulation as allowed) Clinical documentation in EMR systems Outcome measurement and patient progress tracking Knowledge of geriatric conditions and fall prevention Patient and caregiver education Interdisciplinary team collaboration
