Tender Touch Rehab Services seeks a compassionate Physical Therapist Assistant to support delivery of high-quality rehab services for seniors in skilled nursing and long-term care. The PTA will implement treatment plans under the direction of a Physical Therapist, focusing on restoring function, mobility, and independence while honoring each resident's dignity and goals. Responsibilities include therapeutic exercise, gait and balance training, functional mobility, documentation, and family education. Responsibilities Implement physical therapy treatment plans under the supervision of a licensed Physical Therapist Provide therapeutic exercises, gait training, balance and mobility interventions for senior patients Assist with transfer training, ADL-related mobility, and use of assistive devices Observe, measure, and report patient progress, responses, and safety concerns to the PT and care team Accurately document treatments, patient status, and outcomes in the electronic medical record Educate patients and families on exercises, safety, and use of equipment to promote independence Collaborate with an interdisciplinary team, including nursing and other rehab disciplines, to support patient-centered care plans Maintain a safe, clean treatment environment and ensure proper use of equipment Participate in care conferences and contribute to discharge planning and recommendations Engage in ongoing professional development and follow evidence-based practice and regulatory standards Required Skills Therapeutic exercise implementation Gait and balance training Transfer and mobility assistance Use and fitting of assistive devices Geriatric rehabilitation techniques Treatment documentation and EMR use Safe patient handling and fall prevention Patient and family education Interdisciplinary team collaboration Knowledge of rehab regulations and compliance
