Job Description
Tender Touch Rehab Services is seeking a compassionate Physical Therapist Assistant to provide evidence-based rehabilitative care to seniors in skilled nursing and long-term care settings. The PTA will implement individualized treatment plans under PT supervision, focusing on mobility, strength, balance, and functional independence. Responsibilities include direct patient treatment, documentation, patient and family education, and close collaboration with an interdisciplinary care team. Responsibilities Provide rehabilitative treatments under supervision of a Physical Therapist to enhance mobility, strength, and function for seniors Implement individualized care plans and document patient progress accurately and timely Assist with gait training, therapeutic exercises, balance and transfer training, and use of assistive devices Educate patients and families on home exercises, safety, and strategies to maximize independence Collaborate with PTs, nursing, and interdisciplinary team to support patient-centered care goals Ensure compliance with regulatory, facility, and company policies, including documentation and billing standards Maintain a safe, respectful treatment environment that upholds dignity and compassion for all patients Participate in professional development, mentorship, and quality improvement initiatives Required Skills Therapeutic exercise implementation Gait and balance training Neuromuscular re-education Functional mobility and transfer training Use of rehabilitation modalities and equipment Clinical documentation and EMR proficiency Patient and family education Geriatric rehabilitation principles Care planning and interdisciplinary collaboration Knowledge of rehab regulations and reimbursement
