Tender Touch Rehab Services is seeking a compassionate Physical Therapist Assistant to join our Healthcare & Medical Services team supporting seniors in skilled nursing and long-term care settings. The PTA will implement individualized treatment plans designed by the Physical Therapist, focusing on functional mobility, safety, and independence. Responsibilities include gait and balance training, therapeutic exercise, transfers, patient and family education, and timely EMR documentation. You'll collaborate with an interdisciplinary team in a supportive, patient-centered culture grounded in dignity, compassion, and respect. Responsibilities Provide compassionate, evidence-based physical therapy interventions to seniors under the direction of a Physical Therapist Implement individualized treatment plans focused on improving functional mobility, safety, and independence in post-acute and long-term care settings Assist with gait training, transfers, balance, strength, and endurance activities using appropriate therapeutic techniques and equipment Accurately document treatments, patient progress, and outcomes in the electronic medical record in a timely manner Educate patients, families, and caregivers on exercises, safety strategies, and functional techniques to support carryover Collaborate closely with PTs, nursing, and the interdisciplinary team to support patient-centered goals and discharge planning Adhere to all professional, regulatory, and facility standards while upholding patient dignity, compassion, and respect Participate in ongoing professional development, mentorship, and quality improvement initiatives Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility training Safety and fall prevention Use of therapy modalities (heat, cold, e-stim) Patient and family education Documentation and EMR usage Interdisciplinary team collaboration Care plan implementation Knowledge of geriatric conditions and precautions
