Job Description

Legacy Health is seeking a compassionate Physical Therapist Assistant to support patient-centered rehabilitative care in our Portland-area facilities. In this role, you will implement treatment plans designed by Physical Therapists, guide patients through therapeutic exercises, and assist with gait and mobility training. You will document progress, maintain safe and clean treatment areas, and collaborate with an interdisciplinary team. Responsibilities Implement treatment plans under supervision of a Physical Therapist Assist patients with therapeutic exercises and mobility training Monitor and document patient progress and responses to treatment Prepare and maintain treatment areas and equipment Educate patients and families on home exercise programs and safety Collaborate with interdisciplinary care teams to coordinate care Ensure compliance with therapy protocols and safety standards Maintain accurate, timely clinical documentation in EMR Required Skills Therapeutic exercise implementation Gait and balance training Physical therapy modalities (e.g., heat, cold, e-stim) Patient assessment support Clinical documentation and EMRKnowledge of musculoskeletal anatomy Infection control and safety procedures Patient and family education Time management in clinical settings Interdisciplinary team collaboration
