We are a certified home health agency seeking a compassionate and experienced Per Diem Physical Therapist (PT) / Physical Therapist Assistant (PTA) to provide skilled therapy services to patients in their homes. As a key member of the interdisciplinary team, the PT will evaluate patient needs, develop individualized treatment plans, and implement evidence-based interventions to help patients regain mobility, improve function, and achieve optimal outcomes. This role offers flexible scheduling, competitive per-visit compensation, and the rewarding experience of delivering one-on-one care in the comfort of the patient's home.
Responsibilities:
Conduct in-home physical therapy evaluations and reassessments in accordance with the physician's plan of care and Medicare guidelines. Develop and implement individualized therapy plans to restore function, improve mobility, relieve pain, and prevent physical disability. Instruct patients and caregivers in safe use of assistive devices, exercises, and therapeutic techniques to support continued progress. Monitor and document patient response to treatment and modify plans as needed to achieve therapy goals. Collaborate with the interdisciplinary care team (nurses, occupational therapists, speech therapists, aides, and physicians) to ensure coordinated and effective care. Maintain timely, accurate, and compliant documentation in the agency's EMR system. Travel to patient homes throughout the assigned territory
Schedule & Compensation:
Per Diem / PRN - Flexible schedule based on your availability and patient needs. Competitive per-visit rates for initial evaluations, follow-up visits, and reassessments. Current license in the State of Florida. 1+ year of experience as a licensed PT/PTA; home health experience Thorough understanding of Medicare documentation requirements and therapy goals in the home health setting. Proficiency in EMR systems (experience with Kinnser/WellSky or similar is a plus). Excellent interpersonal, communication, and time management skills. Ability to work independently and manage a flexible per diem schedule. Reliable transportation, valid driver's license, and current auto insurance. Why Senior Care Solutions?
