Services of the Physical Therapist Assistant (PTA) will be furnished under the supervision of a physical therapist. The PTA performs services planned, assigned, delegated and supervised by the physical therapist and functions as an active member of the home health team, providing services in accordance with the physician's orders and shall not exceed any of the duties authorized by the license issued by the State for the practice of physical therapist assistant. Duties Assist licensed physical therapists in implementing comprehensive care plans for patients in home health settings, skilled nursing facilities, outpatient clinics, and inpatient environments. Deliver targeted therapies such as trigger point therapy, manual techniques, and therapeutic exercises to improve mobility, strength, and function. Conduct patient assessments, monitor progress, and document responses to treatment accurately and thoroughly. Educate patients and their families on physiotherapy techniques, injury prevention, and discharge planning strategies to promote ongoing health outside of therapy sessions. Support discharge planning efforts by coordinating with healthcare teams to ensure smooth transitions from hospital or facility to home or outpatient care. Maintain detailed records of patient treatments, progress notes, and communication logs in compliance with healthcare regulations. Collaborate with multidisciplinary teams—including nurses, physicians, and social workers—to optimize patient outcomes. Stay current with advancements in physiotherapy practices such as physiology knowledge, anatomy updates, and new therapeutic modalities like trigger point therapy. Qualifications Is licensed by the State Physical Therapist Licensing Board to practice as a Physical Therapy Assistant. Recommend a minimum of one year. Proven experience working in home health environments or skilled nursing facilities; outpatient clinic experience is highly valued. Strong knowledge of anatomy, physiology, medical terminology, and physiotherapy principles. Hands-on experience with diverse treatment modalities including trigger point therapy and orthopedic rehabilitation techniques. Familiarity with inpatient and outpatient care settings across orthopedics, geriatrics, and acute care environments. Ability to develop and follow individualized care plans tailored to each patient's needs. Excellent communication skills for effective patient education and interdisciplinary collaboration.
