Palmetto Subacute Care Center seeks a full-time Physical Therapist Assistant to support individualized rehab programs for post-acute, subacute, and long-term care residents. The PTA will carry out treatment plans under PT supervision, providing therapeutic exercises, gait training, balance and strengthening activities, and functional mobility interventions. Responsibilities include tracking outcomes, documenting progress, educating residents and families, and collaborating with an interdisciplinary team in a compassionate, resident-centered environment focused on recovery, comfort, and quality of life. Responsibilities Provide direct patient care under the supervision of a licensed Physical Therapist Implement treatment plans focused on mobility, strength, and functional independence Assist residents with therapeutic exercises, gait training, and use of assistive devices Observe, measure, and document patient progress and report findings to the PT and care team Educate patients and families on exercises, safety, and rehabilitation goals Collaborate with nursing, rehab, and interdisciplinary teams to support resident-centered care Ensure proper use and maintenance of therapy equipment and treatment areas Uphold regulatory, safety, and infection control standards in all therapy activities Required Skills Therapeutic exercise implementation Gait training and balance therapy Functional mobility training Patient assessment and observation Electronic medical record documentation Therapeutic modalities (heat, cold, ultrasound) Assistive device fitting and training Interdisciplinary care coordination Patient and family education Knowledge of rehab and long-term care regulations
