PACE Place provides Medical, Rehabilitation, In-Home Assistance, Transportation, Social Work, and Day Center services for patients in our care. Position Summary The PACE Physical Therapy Assistant (PTA) administers physical therapy to participants on an intermittent basis in either at the PACE Day Center or at their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Center Manager or Physical Therapist (PT).
Shift:
Mon - Fri Essential Functions Assists Therapists and provides maintenance care Maintains supplies and equipment and the Physical Therapy Department in a clean and orderly manner Provides supervision to program aides and other staff who are implementing a physical therapy treatment program Assists Therapists and other PTA's in treatment Provides maintenance care to participants per Interdisciplinary Team (IDT) plan of care Prepares participants for treatments Responsible for routine cleaning and maintenance of department equipment and supplies Documents pertinent participant/treatment activities per PACE Partners policies Understands and adheres to established PACE Partners policies and procedures Provides physician prescribed physical therapy under a plan of care established by the Clinic Manager and/or PT Improves or minimizes residual physical disabilities of the participant Returns the individual to optimum and productive level within the participant's capabilities Participates with all other health care personnel in participant care planning Performs all skilled procedures as ordered by physician and according to the plan of care established by the Clinic Manager and/or PT Consults with Clinic Manager and/or PT regarding change in treatment Instructs participants and family members on in-home programs and activities of daily living Participates in in-service programs and presents in-service programs as assigned Participates in QIPI activities as assigned Attends all participant care conferences as scheduled Prepares medical records, progress notes, and updates care plans for each participant visit in a timely manner as per PACE Partners policy Performs services planned, delegated, and supervised by the Clinic Manager or PT Assists in preparing clinical and progress notes Participates in educating the participant and family Comply with all state and/or federal regulations to ensure all complaints regarding quality of care or service delivery are properly processed as a grievance. Comply with all state and/or federal regulations to ensure all individual issues within a grievance are reviewed, considered, and resolved. Comply with all state and/or federal regulations to ensure all participant medical records are complete, accurate and accessible to all staff. Only act within the scope of his or her authority to practice Other duties as assigned Qualifications Knowledge and Critical Skills Strong communication skills, including face-to-face, telephone, and computer skills. Must be able to interact successfully with participants, families, and other agencies to present a positive and professional image Ability to manage multiple priorities simultaneously and effectively handle the emotional stress of the workload Knowledge of the principles and practice of primary medical care Demonstrated knowledge and skills necessary to provide care to and communicate with PACE Partners participants Demonstrated knowledge of the principles of growth and development over the life span Ability to work collaboratively with PACE Partners employees as part of the daily IDT meetings Ability to adapt to participant status and needs as directed Ability to accommodate schedule changes daily due to staffing, participant condition, needs, and new participants Solid Microsoft Office skills including Word, Excel, PowerPoint, and Outlook Experience Have 1 year of experience working with a frail or elderly population OR, if the individual has less than 1 year of experience but meets all other job requirements, the candidate must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring. Meet a standardized set of competencies for the specific position description established by the PACE organization before working independently. Restorative care experience preferred Ability to use equipment with visual and auditory mechanisms License / Certification Florida Physical Therapist Assistant (PTA) License CPR with First Aid or BLS certification required Education Florida Physical Therapist Assistant (PTA) License Other Requirements Valid Florida Driver's License and Automobile Insurance Reliable transportation to meet visit schedule The knowledge, skills and abilities listed above are typically acquired through the levels of education and experience listed.
