Job Description

Palmetto Subacute Care Center seeks a full-time Physical Therapist Assistant to support high-quality rehab services in a skilled nursing setting. The PTA will deliver compassionate, resident-centered care, assisting with therapeutic exercises, mobility, balance, and strengthening under PT supervision. Responsibilities include carrying out treatment plans, tracking and documenting progress, preparing equipment, and reinforcing patient and family education. The role works closely with an interdisciplinary team, supporting recovery, comfort, and quality of life in a collaborative, clinically focused environment. Responsibilities Provide direct patient care under supervision of licensed Physical Therapist following individualized treatment plans Assist with therapeutic exercises, gait training, balance and strength activities for post-acute, subacute, and long-term care residents Observe, document, and report patient progress, responses to treatment, and any changes in condition Prepare treatment areas, equipment, and assistive devices to ensure safe and efficient therapy sessions Educate and encourage residents on home exercise programs, mobility, and safe functional activities Collaborate with therapists, nursing, and interdisciplinary team to support resident-centered care plans Maintain accurate, timely documentation in accordance with facility and regulatory standards Follow infection control, safety, and compliance policies in all therapy activities Required Skills Therapeutic exercise implementation Gait training and mobility assistance Balance and neuromuscular re-education Use of assistive devices and DMEPatient transfer and safe handling techniques Skilled nursing/long-term care rehab Treatment documentation and EMR use Patient and family education Vital sign monitoring and observation Infection control and safety compliance
