Tender Touch Rehab Services is seeking a compassionate Physical Therapist Assistant to join our Healthcare & Medical Services team serving seniors in skilled nursing and long-term care. The PTA will implement PT treatment plans focused on mobility, strength, balance, and functional independence, delivering patient-centered, evidence-based care. Responsibilities include providing one-on-one therapy, documenting progress, educating patients and families, and collaborating with an interdisciplinary team. Responsibilities Implement treatment plans under the direction of a Physical Therapist for senior patients in skilled nursing and long-term care settings. Assist patients with therapeutic exercises, gait training, balance, and functional mobility to improve safety and independence. Accurately document treatments, patient progress, and outcomes in the EMR in a timely, compliant manner. Educate patients, families, and caregivers on exercises, safety, and strategies to maximize function and quality of life. Collaborate with the interdisciplinary healthcare team to support patient-centered care plans and discharge goals. Maintain a safe, clean treatment environment and follow all infection control, regulatory, and company policies.
