Job Description

Solaris HealthCare is seeking a compassionate Certified Physical Therapy Assistant to provide high-quality rehab services in our skilled nursing and long-term care setting. In this onsite role, you will deliver therapeutic exercises, gait and balance training, and functional mobility interventions under the supervision of a Physical Therapist. You'll document progress in the EMR, collaborate with nursing and rehab teams, and educate residents and families. Responsibilities Deliver physical therapy interventions under supervision of a licensed Physical Therapist Implement individualized treatment plans to improve mobility, strength, and function Accurately document treatments, progress notes, and patient responses in EMRAssist with evaluations by gathering patient history and performing delegated tests Educate residents and families on exercises, safety, and use of assistive devices Collaborate with nursing, rehab, and interdisciplinary teams to support care goals Ensure compliance with clinical standards, regulations, and facility policies Maintain a safe, clean therapy environment and properly use equipment Support discharge planning with home exercise and mobility recommendations Uphold a patient-centered, dignified, and compassionate care environment Required Skills Therapeutic exercise implementation Neuromuscular re-education Gait and balance training Manual therapy techniques (within PTA scope) Use of therapy and rehab equipment Electronic medical record (EMR) documentation Vital signs monitoring and patient observation Care planning with PTs and IDTSafe patient handling and transfer techniques Patient and family education
