Physical Therapist Assistant (PTA) - Full-Time & Per Diem Comfort Creek Nursing & Rehab Center 10200 US Highway 1 South Wadley, GA 30477 Job Overview Comfort Creek Nursing & Rehab Center is seeking a dedicated and compassionate Physical Therapist Assistant (PTA) to join our in-house rehabilitation team . We are currently hiring for both Full-Time and Per Diem (PRN) positions. The PTA will work under the direction and supervision of a licensed Physical Therapist to provide high-quality rehabilitation services to residents in our skilled nursing and subacute rehabilitation setting. This role offers an opportunity to make a meaningful impact by helping residents regain strength, mobility, independence, and quality of life following illness, injury, surgery, or hospitalization. Essential Duties and Responsibilities Administer physical therapy treatments and interventions as outlined in the Physical Therapist's plan of care. Assist residents in improving mobility, strength, balance, endurance, and functional abilities. Monitor resident response to treatment and communicate progress or concerns to the supervising Physical Therapist. Accurately document treatment sessions, patient performance, and progress toward goals in accordance with facility and regulatory requirements. Collaborate with interdisciplinary team members including nursing, therapy, social services, and physicians to optimize resident outcomes. Educate residents, families, and caregivers regarding therapy programs and safety recommendations. Participate in care plan meetings and discharge planning as appropriate. Maintain compliance with Medicare, Medicaid, state, federal, and facility regulations. Support a positive, patient-centered rehabilitation environment. Qualifications Required Graduate of an accredited Physical Therapist Assistant program. Current Physical Therapist Assistant license in the State of Georgia. Current CPR certification or willingness to obtain upon hire. Excellent communication, organizational, and interpersonal skills. Ability to work effectively as part of a multidisciplinary rehabilitation team. Preferred Experience in skilled nursing facilities, subacute rehabilitation, long-term care, or post-acute care settings. Familiarity with Medicare documentation and reimbursement guidelines. Experience treating geriatric and medically complex patient populations.
