LTC/Nursing Home (SNF/ICF) This travel position supports a long-term care/ skilled nursing facility environment, delivering rehabilitation services under supervision within Hawaii-relevant practice standards.

Long-term care / skilled nursing facility (SNF/ICF) Training on onboarding: departmental orientation and EMR documentation training provided Responsibilities Provide rehabilitation services in a SNF/ICF setting, including initial evaluations, reassessments, treatments, and related clinical services within licensure scope. Develop, implement, and carry out individualized treatment plans in collaboration with the supervising therapist, physician, and interdisciplinary team as appropriate to the role. Implement evidence-based therapeutic interventions to maximize function, mobility, communication, and overall quality of life. Educate residents, families, and caregivers regarding treatment plans, progress, and discharge planning; document progress and changes in the plan of care per facility and regulatory requirements. Complete accurate, timely documentation in the EMR, including progress notes, treatment changes, and discharge information; supervise therapy assistants when applicable. May be required to float to other units or facilities based on census and staffing needs; be available to work Hawaii state and federal holidays as necessary. Requirements Current Hawaii State license in good standing appropriate to the discipline; Hawaii license must be active by start of assignment. Current Basic Life Support (BLS) certification. Minimum two (2) years of SNF/ICF experience preferred. Ability to work in a SNF/ICF setting and follow facility workflows and safety guidelines. Training on EMR systems and interdisciplinary collaboration as part onboarding.
