Your Responsibilities:
Assist Physical Therapists in establishing and administering treatment plans with specific therapy goals. Help assess patient needs; plan, evaluate, and modify care to meet physical therapy intervention goals. Collaborate with all disciplines to plan and evaluate team goals for each patient. Support in setting realistic and achievable goals for patients/residents. Document and record patient/resident condition and progress per state and federal regulations. Educate patients and families in appropriate therapy methods. Optimize patient functionality and skills by assisting in the screening of all residents. Communicate patient status and needs to the patient, family, and other involved professionals.
