Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support seniors in skilled nursing and post-acute care settings. Working under the direction of a Physical Therapist, you will implement individualized treatment plans, assist with therapeutic exercises and mobility training, and document patient progress. Responsibilities Implement physical therapy treatment plans under the supervision of a licensed Physical Therapist Assist patients with therapeutic exercises, gait training, and functional mobility activities Monitor, record, and report patient progress and response to treatment Ensure accurate, timely clinical documentation in accordance with facility and regulatory standards Educate patients and families on exercises, safety, and functional goals Collaborate with an interdisciplinary care team to optimize functional outcomes Maintain a safe, clean treatment environment and follow infection control protocols Support discharge planning and recommendations for continued services or equipment needs Required Skills Therapeutic exercise implementation Gait training and balance interventions Functional mobility training Knowledge of geriatric rehabilitation principles Skilled nursing/post-acute care experience Clinical documentation in EMR systems Patient and family education Transfer techniques and safe patient handling Knowledge of Medicare and rehab compliance guidelines Outcome measurement and progress tracking
