Job Description

Renewal Rehab is seeking a compassionate Physical Therapist Assistant to join our Healthcare & Medical Services team serving seniors in skilled nursing and post-acute care settings. In this role, you will deliver high-quality rehabilitative treatments under the direction of a Physical Therapist, focusing on mobility, strength, balance, and safe functional independence. You'll work in a collaborative, patient-centered culture with strong clinical resources, mentorship, and open communication. Responsibilities Deliver PTA treatments under the direction of a Physical Therapist for senior patients in skilled nursing and post-acute settings Implement individualized care plans to improve mobility, strength, balance, and functional independence Accurately document treatments, patient progress, and outcomes in the EMR in a timely manner Educate patients, families, and caregivers on exercises, safety, and functional strategies for home and facility settings Collaborate with an interdisciplinary team to support patient-centered goals and discharge planning Contribute to a culture of integrity, respect, and clinical excellence in senior care Participate in mentorship, in-services, and ongoing professional development offered by Renewal Rehab Required Skills Therapeutic exercise implementation Gait training Functional mobility training Use of modalities (heat, cold, e-stim as allowed) Documentation in EMR systems Patient and family education Care plan implementation Balance and fall-prevention techniques Vital signs monitoring Interdisciplinary team collaboration
