Job Description
Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support personalized rehab programs for seniors in skilled nursing and post-acute settings. In this role, you'll deliver hands-on treatments under the supervision of a Physical Therapist, focusing on improving mobility, safety, and independence. You'll assist with evaluations, follow individualized care plans, document progress, and educate patients and families. Responsibilities Provide direct patient care under the supervision of a Physical Therapist, following individualized treatment plans. Implement therapeutic exercises, gait training, balance activities, and functional mobility interventions. Assist with patient assessments and accurately document treatments, progress, and outcomes in the EMR system. Educate patients and families on exercises, safety, and strategies to support functional independence. Collaborate with an interdisciplinary team to support patient-centered goals and discharge planning. Adhere to clinical, regulatory, and safety standards, including infection control and ethical practice. Participate in mentorship, in-service training, and continuing education offered by Renewal Rehab. Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility training Patient assessment and documentation (EMR) Knowledge of geriatric rehab protocols Therapeutic modalities (e.g., heat, e-stim) Care plan execution under PT direction Patient and family education Infection control and safety practices Interdisciplinary team collaboration
