Job Description

Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support seniors in skilled nursing and post-acute care settings. In this role, you will implement treatment plans under the direction of a Physical Therapist to help patients improve mobility, strength, and independence. You will provide one-on-one, patient-centered care, document progress, and collaborate closely with an interdisciplinary team. Renewal Rehab offers a supportive, mentoring environment, manageable caseloads, and opportunities for growth while you make a meaningful daily impact on seniors' quality of life. Responsibilities Deliver PTA treatments under the direction of a Physical Therapist for senior patients in skilled nursing and post-acute care settings. Implement individualized treatment plans focused on improving mobility, strength, balance, and functional independence. Accurately document treatments, patient progress, and outcomes in the electronic medical record in a timely manner. Educate patients, families, and caregivers on home exercises, safety strategies, and functional mobility techniques. Collaborate closely with PTs, nurses, and the interdisciplinary team to support patient-centered care plans. Participate in care conferences and contribute to discharge planning and goal setting for patients. Maintain compliance with regulatory, ethical, and company standards, ensuring high-quality, evidence-based care. Support a positive, respectful clinic environment that reflects Renewal Rehab's integrity and senior-focused mission. Required Skills Therapeutic exercise implementation Gait training and balance interventions Functional mobility and transfer training Use of therapy modalities (e.g., heat, cold, e-stim) per PT plan Documentation in EMR/therapy software Care planning in skilled nursing/post-acute settings Patient and family education Safety assessment and fall prevention Interdisciplinary team collaboration Understanding Medicare and SNF therapy guidelines
