Job Description

Renewal Rehab is seeking a Certified Physical Therapist Assistant to support seniors in skilled nursing and post-acute care settings. In this role, you will deliver compassionate, evidence-based interventions under the direction of a Physical Therapist to improve mobility, function, and quality of life. You will treat manageable caseloads, document in an EMR, and collaborate with an interdisciplinary team. Renewal Rehab offers a patient-centered, collaborative culture with strong clinical resources, mentorship, and opportunities for professional growth in senior care. Responsibilities Deliver physical therapy interventions under the direction of a licensed Physical Therapist for seniors in skilled nursing and post-acute care. Implement individualized treatment plans focusing on functional mobility, strength, balance, and safety. Accurately document treatments, progress notes, and outcomes in the EMR in a timely, compliant manner. Educate patients, families, and caregivers on home exercise programs and safe mobility strategies. Collaborate with an interdisciplinary care team to support patient-centered goals and discharge planning. Monitor patient response, report changes to the PT, and recommend adjustments to the plan of care. Contribute to a positive, respectful environment aligned with Renewal Rehab's values of integrity and quality outcomes.
