Renewal Rehab is seeking a compassionate Physical Therapist Assistant to provide high-quality rehab services for seniors in skilled nursing and post-acute care settings. Working closely with a supervising PT, you will implement individualized treatment plans to improve mobility, safety, and functional independence. Responsibilities include delivering therapeutic exercises and functional training, documenting progress, educating patients and families, and collaborating with an interdisciplinary team. Responsibilities Deliver skilled PTA treatments under the direction of a licensed Physical Therapist for seniors in skilled nursing and post-acute care facilities. Implement individualized care plans focused on mobility, strength, balance, and fall prevention to enhance safety and independence. Accurately document treatments, progress, and patient responses in the EMR, following regulatory and payer requirements. Educate patients, families, and caregivers on exercises, safety strategies, and functional techniques to support carryover. Collaborate with PTs, nursing, and the interdisciplinary team to adjust plans and support quality clinical outcomes. Participate in case conferences and contribute to discharge planning and recommendations for continued services. Adhere to company, state, and federal guidelines, maintaining high ethical and clinical standards in senior care. Support a collaborative, patient-centered culture through respectful communication and mentorship participation.
