Job Description

Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support therapy services for seniors in skilled nursing and post acute care settings. As a PTA, you will implement individualized treatment plans under the direction of a Physical Therapist, focusing on mobility, balance, strength, and functional independence. You'll collaborate with an interdisciplinary team, document progress, educate patients and caregivers, and help ensure safe discharges. Responsibilities Implement PT treatment plans under supervision of a licensed Physical Therapist Provide therapeutic exercises, gait training, and balance activities for seniors Assist with transfer training and functional mobility to improve independence Monitor, document, and report patient progress and responses to treatment Educate patients, families, and caregivers on home exercise and safety strategies Collaborate with nursing, rehab, and medical teams on care planning and discharge needs Ensure compliance with state, federal, and payer documentation requirements Maintain safe, clean treatment areas and properly use therapy equipment Contribute to fall-prevention and functional maintenance programs Participate in in service training and quality improvement initiatives Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility and transfer training Geriatric rehabilitation techniques Skilled nursing/post acute rehab procedures Rehabilitation documentation and EMR use Patient and caregiver education Knowledge of PT modalities and equipment Understanding of Medicare and payer guidelines Interdisciplinary team collaboration
