Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support delivery of high-quality rehabilitation services for seniors in skilled nursing and post-acute care settings. The PTA will implement treatment plans under the direction of a Physical Therapist, focusing on improving mobility, strength, balance, and functional independence. Responsibilities include assisting with patient assessments, leading therapeutic exercises, documenting progress, collaborating with an interdisciplinary team, and educating patients and families. Responsibilities Implement physical therapy treatment plans under the supervision of a licensed Physical Therapist Provide therapeutic exercises, gait training, balance and functional mobility interventions Assist with patient observations and report changes in status to the supervising PT and care team Document treatments, patient progress, and outcomes in the electronic medical record accurately and timely Educate patients and families on home exercise programs and safety strategies Collaborate with nursing, rehab, and interdisciplinary team members to optimize functional outcomes Ensure treatments follow regulatory, ethical, and company quality standards Assist with discharge planning and recommendations for continued services or equipment needs Maintain a safe treatment environment and proper use of rehab equipment Participate in in-services, mentorship, and continuing education to support professional growth Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility training Geriatric rehabilitation Skilled nursing/post-acute care experience Patient and family education Electronic medical record documentation Safety and fall prevention Transfer and body mechanics techniques Interdisciplinary team collaboration
