Job Description
Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support high-quality rehabilitation services for seniors in skilled nursing and post-acute care settings. Working under the direction of a Physical Therapist, you will implement individualized treatment plans, assist with therapeutic exercises and mobility training, and document patient progress. You'll collaborate closely with an interdisciplinary team to promote functional independence, safety, and meaningful outcomes. Responsibilities Implement physical therapy treatment plans under the direction of a licensed Physical Therapist Assist patients with therapeutic exercises, gait training, and functional mobility activities Monitor, document, and report patient progress and response to treatment Educate patients, families, and caregivers on safe mobility and home exercise programs Collaborate with an interdisciplinary team to support individualized care plans Ensure treatments follow regulatory, ethical, and company standards for senior care Maintain accurate, timely clinical documentation in the electronic medical record Assist with discharge planning and recommendations for continued services or equipment Contribute to a positive, patient-centered environment focused on quality outcomes Participate in ongoing training and professional development opportunities Required Skills Therapeutic exercise implementation Gait training and balance interventions Transfer and mobility training Geriatric rehabilitation techniques Skilled nursing and post-acute care familiarity Electronic medical record (EMR) documentation Patient and caregiver education Vital signs monitoring and safety assessment Knowledge of PT plan of care execution Understanding of regulatory and billing requirements for therapy services
