Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support high quality rehabilitation services for seniors in skilled nursing and post acute care settings. Working closely with Physical Therapists, you'll carry out treatment plans, assist with therapeutic exercises and mobility training, document patient progress, and help promote safety and independence. Responsibilities Implement PT treatment plans under direction of licensed Physical Therapist Assist with therapeutic exercises, gait and balance training, and functional mobility Monitor, document, and report patient progress and response to treatment Educate patients and caregivers on exercises, safety, and use of assistive devices Maintain accurate, timely, and compliant clinical documentation Collaborate with interdisciplinary care team to support patient goals and discharge planning Ensure safe use of equipment and adherence to infection control and facility policies Contribute to a positive, respectful, and patient-centered care environment Participate in in-service training and ongoing professional development Support efficient scheduling and manageable caseload management Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility training Patient and caregiver education Clinical documentation (EMR) Knowledge of geriatric conditions and precautions Transfer and wheelchair skills Safety and infection control practices Use of assistive devices and DMEInterdisciplinary team communication
