Job Description

Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support skilled nursing and post-acute care residents. In this role, you will implement treatment plans under the direction of a Physical Therapist, focusing on improving mobility, strength, balance, and functional independence for seniors. Collaborate closely with an interdisciplinary care team, document patient progress, and educate residents and families. Responsibilities Implement PT treatment plans for senior patients under supervision of a Physical Therapist Assist with therapeutic exercises, gait training, balance and mobility interventions Monitor, document, and report patient progress and response to treatment Educate patients and families on exercises, safety, and use of assistive devices Collaborate with an interdisciplinary team to support care plans and discharge goals Ensure compliance with regulatory, documentation, and facility policies Maintain a safe, clean, and supportive treatment environment Contribute to care conferences and participate in quality improvement initiatives Required Skills Therapeutic exercise implementation Gait training and balance interventions Functional mobility training PT documentation and EMR use Geriatric rehabilitation techniques Safety and fall prevention Use of assistive devices and DMEPatient and family education Vital sign monitoring during activity Regulatory and compliance awareness in SNF settings
