Renewal Rehab seeks a compassionate Physical Therapist Assistant to support seniors in skilled nursing and post-acute care settings. Under PT supervision, you will deliver evidence-based treatments, assist with evaluations, and help patients improve mobility, strength, and functional independence. You'll document progress, educate patients and caregivers, and collaborate with an interdisciplinary team. Responsibilities Provide direct physical therapy interventions under PT supervision for senior patients in skilled nursing and post-acute care. Assist with comprehensive patient evaluations, reassessments, and care plan implementation focused on functional outcomes. Document treatments, patient progress, and billing information accurately and timely in accordance with regulations. Educate patients, families, and caregivers on exercises, mobility, and safe functional activities. Collaborate with the interdisciplinary team to support patient-centered care and discharge planning. Maintain compliance with state, federal, and facility regulations and Renewal Rehab policies. Contribute to a positive, respectful, and collaborative clinical culture focused on quality outcomes.
