Renewal Rehab seeks a compassionate Physical Therapist Assistant to support high-quality rehabilitation services for seniors in skilled nursing and post-acute care settings. Working under the direction of a Physical Therapist, you will implement treatment plans, lead therapeutic exercises, document progress, and collaborate with an interdisciplinary team to improve mobility, safety, and independence. Responsibilities Implement PT treatment plans under supervision of a Physical Therapist Guide patients through therapeutic exercises and functional mobility training Assist with balance, gait, and transfer training for senior patients Accurately document treatments, patient responses, and progress in EMRCommunicate with PTs, nurses, and facility staff on patient status and goals Educate patients and families on home exercises and safety strategies Assist with care plan development and goal progression for rehab patients Ensure safe use of equipment and adherence to infection control protocols Participate in interdisciplinary team meetings and discharge planning Support a patient-centered, respectful, and compliant care environment Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility training Geriatric rehabilitation techniques Skilled nursing/post-acute rehab practices Patient and family education Clinical documentation and EMR use Safety and transfer techniques Range of motion and strengthening interventions Infection control and standard precautions
