Job Description
Renewal Rehab is seeking a caring Physical Therapist Assistant to join our team serving seniors in skilled nursing and post-acute care. In this role, you'll deliver individualized treatment plans under the direction of a Physical Therapist, focusing on improving mobility, safety, and independence. You'll perform therapeutic exercises, gait and balance training, and functional activities while documenting progress in our EHR system. Working in a collaborative, patient-centered culture, you'll benefit from mentorship, ongoing education, and clear pathways for professional growth. Responsibilities Provide physical therapy interventions under the direction of a licensed Physical Therapist Implement individualized treatment plans focused on mobility, strength, balance, and functional independence Deliver gait training, therapeutic exercises, and functional activities for seniors in skilled nursing and post-acute care Accurately document treatments, progress notes, and outcomes in the electronic health record (EHR) system Educate patients and families on home exercise programs, safety, and strategies to maintain gains Collaborate with an interdisciplinary team, including PTs, OTs, nurses, and facility staff, to coordinate patient care Monitor patient response to treatment and report changes or concerns to the supervising Physical Therapist Ensure compliance with regulatory, facility, and professional practice standards Contribute to a positive, patient-centered environment aligned with Renewal Rehab's mission and values Participate in mentorship, in-service training, and continuing education opportunities to advance clinical skills Required Skills Therapeutic exercise and functional training Gait and balance training Treatment planning and progression documentation Use of rehab equipment and modalities Electronic health record (EHR) documentation Patient and family education Interdisciplinary team collaboration Knowledge of geriatric conditions and fall prevention Outcome tracking and goal setting Regulatory and documentation compliance in SNF/post-acute
