Renewal Rehab is seeking a compassionate Physical Therapist Assistant to provide hands-on care to seniors in skilled nursing and post-acute care settings. In this on-site role, you will deliver therapeutic exercises, gait training, and functional mobility interventions under the direction of a Physical Therapist, focusing on safety, independence, and quality of life. You'll document patient progress, collaborate with an interdisciplinary team, and support patient and family education. Responsibilities Deliver PTA interventions under the direction of a licensed Physical Therapist to senior residents in skilled nursing and post-acute settings. Assist with functional mobility, gait training, therapeutic exercise, and balance activities to improve safety and independence. Document treatments, patient progress, and responses in the EMR in accordance with regulatory and facility requirements. Collaborate with an interdisciplinary team and participate in care planning to support patient-centered goals. Educate patients and families on exercises, safety strategies, and use of assistive devices to promote carryover. Contribute to a positive, respectful, and collaborative clinical environment focused on quality outcomes.
