Job Description
Renewal Rehab seeks a compassionate Physical Therapist Assistant to support seniors in skilled nursing and post-acute settings. In this role, you'll deliver hands-on therapy under the direction of a Physical Therapist, focusing on mobility, strength, and functional independence. You'll collaborate within an interdisciplinary team, maintain accurate documentation, and educate patients and caregivers. Responsibilities Provide direct physical therapy interventions under PT supervision for senior patients in skilled nursing and post-acute settings Assist with treatment planning by observing, documenting, and reporting patient progress and response to therapy Instruct patients and caregivers in exercises, mobility techniques, and use of adaptive equipment to improve safety and function Maintain accurate, timely clinical documentation in accordance with company, facility, and regulatory standards Collaborate with PTs, nurses, and interdisciplinary team members to support patient-centered care plans Ensure safe use, cleaning, and organization of therapy equipment and treatment areas Participate in care conferences and discharge planning as needed Support quality improvement initiatives and evidence-based practice within the therapy team Required Skills Therapeutic exercise implementation Gait training and balance interventions Functional mobility training (transfers, bed mobility) Use of therapy modalities and equipment (e.g., walkers, parallel bars) Clinical documentation in EMR systems Patient and caregiver education Geriatric and post-acute rehabilitation techniques Pain management techniques within PT scope Care planning within an interdisciplinary team Knowledge of Medicare and reimbursement guidelines
