Job Description

Renewal Rehab seeks a compassionate Physical Therapist Assistant to deliver high-quality, evidence-based care for seniors in skilled nursing and post-acute settings. Working under the direction of a Physical Therapist, you will implement individualized treatment plans to improve mobility, strength, balance, and safety, while promoting dignity and independence. You'll collaborate closely with an interdisciplinary team, maintain accurate documentation, and educate patients and families. Responsibilities Provide evidence-based physical therapy interventions under PT supervision for seniors in skilled nursing and post-acute settings Assist with evaluations, implement treatment plans, and progress functional mobility and strength Document treatments, patient progress, and outcomes accurately in EMR systems Educate patients, families, and caregivers on exercises, safety, and functional goals Collaborate with an interdisciplinary team to optimize patient outcomes and discharge planning Support a patient-centered, compassionate care environment aligned with Renewal Rehab's values Required Skills Therapeutic exercise implementation Functional mobility training Gait and balance training Neuromuscular re-education Therapeutic modalities application Patient and family education Geriatric rehabilitation techniques Care planning and documentation (EMR) Interdisciplinary care coordination Safety and fall prevention strategies
